Chapter 1 Rain over Medellín

The sky over the Aburrá Valley didn't merely pour it collapsed. Rain fell in thick, unrelenting sheets, drowning the sprawling grid of Medellín in a suffocating gray haze. It was the kind of storm that cleansed the streets of lingering heat while washing the sins of the underworld down into the roaring gutters.

High above the city's concrete heart, the heavy, armored chassis of a black Mercedes Benz G Wagon cut through the torrential night like a shark gliding through dark water.Inside the leather-trimmed cabin, silence reigned a silence heavier than the thunder shaking the mountainside. At the wheel sat Don Matteo El Hielo Restrepo. At thirty two, his name was spoken in whispered, terrified tones across the Antioquia department. He was a man crafted from hard angles and colder resolve, earned through a decade of surviving the bloody power vacuums left behind by old guard narco lords. To the street soldiers of Comuna 13, he was a ghost, to his rivals in Cali and Bogotá, he was an untouchable strategist whose heart was rumor rather than fact.Matteo adjusted his grip on the leather steering wheel, his knuckles whitening slightly as the heavy wiper blades struggled against the downpour. In the passenger seat, his primary enforcer, Esteban, sat with an assault rifle resting casually across his lap, his eyes scanning the gloom through night vision goggles. We should have taken the highway, Boss, Esteban muttered, his radio crackling with low static from the tailing security detail. The lower roads near the river freeze up with mud during downpours like this. It's an ideal spot for an ambush.Let them try, Matteo replied, his voice a low, gravelly baritone that carried no warmth. The Cárdenas cartel is too busy burying their commanders from last night's sweep to set an ambush on my turf. Watch the mirrors, Esteban. Nothing more.Matteo's mind was elsewhere. He was calculating logistics shipments moving through the Caribbean ports, bribes destined for local politicians, and the quiet, persistent rumor that someone within his inner council had begun leaking supply routes to federal authorities. In his line of work, distraction was a death sentence. He eased his foot onto the accelerator as the heavy SUV descended a winding slope lined with dense jungle foliage on one side and crumbling brick dwellings on the other. The headlights pierced through the deluge, catching the glint of overflowing drainpipes and slick asphalt.

Then, the world shattered into chaos.

A figure darted out from the dark periphery of the road not a rival gunman, not a street enforcer, but a young woman running blindly through the storm. She was drenched, her dark hair plastered against her face, her hands raised futilely against the blinding beams of the LED headlights.Look out! Esteban yelled, his hand slamming against the dashboard.Matteo reacted instantly. His elite tactical training took over as he slammed his foot onto the brake pedal and turned the wheel sharply to the left. The four ton armored vehicle skidded across the hydroplaning road, its tires screeching against the slick asphalt like a dying animal. The heavy vehicle shuddered violence into the frame, but physics proved merciless on the wet slope. The front right bumper clipped her with terrifying force.A sickening, metallic thud echoed above the sound of the engine, followed by the soft, terrible sound of a body hitting the rain soaked gravel before sliding into the dark gutter at the edge of the jungle. The Mercedes slammed into an embankment, the engine stalling out with a sharp hiss of steam from the radiator. Inside the vehicle, the sudden silence was deafening, broken only by the rhythmic thrum of heavy rain beating against the reinforced glass.Dammit! Esteban cursed, flicking off his safety and unbuckling his belt in one fluid motion. Was it a hit? A distraction?

Stay inside, Matteo commanded, his voice cold, though a sudden, unfamiliar surge of adrenaline spiked in his veins. He unfurled his long frame, opened his door, and stepped out into the freezing deluge. The rain hit him like needles, soaking his tailored coat within seconds. He pulled a heavy tactical pistol from his shoulder holster, keeping it pointed down as his polished boots splashed through deep puddles toward the ditch. His tailing escort vehicles pulled up behind him, their high beams illuminating the scene in stark, harsh white light. Tactical team members spilled out, forming a defensive perimeter with weapons raised toward the surrounding tree line.Matteo paid them no attention. He slid down the muddy slope into the gutter.A young woman lay twisted in the mud. Her pale skin stood out against the dark earth, her yellow sundress torn and ruined by grease and gravel. Her breathing was shallow, ragged, and wet a sound Matteo had heard a hundred times on the battlefield, but never like this.He knelt beside her, sinking his tailored trousers straight into the mud without a second thought. For a man whose hands had ordered the deaths of dozens, the sight of this innocent, broken civilian sparked a strange suffocating knot of guilt in his chest.Boss Esteban called down, his gun trained on the darkness. We need to clear the area. If the feds or rival spotters see this vehicle.Shut up Matteo growled discarding his weapon into his pocket. He slid his hand beneath her neck, supporting her head as he brushed drenched strands of dark hair away from her face. The light from the escort trucks caught her features clearly for the first time.

Matteo froze. His breath hitched in his throat freezing in the damp air.Her face was pale, delicate, framed by high cheekbones and dark lashes resting against wet skin. She couldn't have been more than twenty two. But it wasn't her youth or her beauty that stopped the heart of Medellín's most feared cartel boss it was the striking, undeniable familiarity of her face. He had seen those exact eyes, that same jawline, in a dossier he had burned six months ago.

She wasn't just a stranger on a dark road. She was connected to a ghost from his past a dead man whose blood was permanently stamped onto Matteo's soul. The young woman groaned softly, her chest heaving as she coughed up a small trace of blood. Her hand, trembling and covered in road grime, reached out blindly into the dark, her fingers brushing against the cold leather of Matteo's jacket.Help me she whispered, her voice barely audible over the roar of the storm.Matteo looked down at her fingers gripping his sleeve. His mind raced, calculating the catastrophic risks of what he was about to do. Taking her to a public hospital would mean police reports, investigations, and spotlighting his location to every cartel hunting his head. Leaving her meant she would bleed out in the mud within twenty minutes. He scooped her up into his arms, lifting her fragile frame effortlessly against his chest. Her head slumped against his shoulder, her faint, warm breath brushing against his neck.Boss, What are you doing? Esteban shouted, stepping down the slope. We leave her for the local ambulance We can't take a civilian.Matteo marched back up the mud slicked bank, his grip tightening around the bleeding girl as the storm raged around them.

Call Dr. Vargas, Matteo barked over his shoulder, his eyes locked ahead as he carried her toward the back door of his secondary escort SUV. Tell him to prepare the surgical room at the hacienda immediately. At the hacienda? Esteban stared at him in disbelief. Boss, that's a security breach We don't even know who she is.

Matteo opened the door, laid her gently across the leather rear seat, and leaned over her trembling body. He looked down at her pale, shivering face, his heart hammering against his ribs for the first time in years.I know exactly who she is,Matteo murmured softly to himself, his voice swallowed by the thunder above. He climbed into the vehicle, slammed the door shut, and pulled the bleeding girl into his lap as the convoy sped off into the dark, storm-drenched night.

Chapter 2 Cold hands, warm pulse

Chapter 2: Cold Hands, Warm Pulse

The convoy tore through the blackness of the mountain highway its tires cutting through the rising floodwaters like surgical blades. Inside the rear cabin of the lead escort SUV the atmosphere was suffocatingly tense. The scent of rain ozone and rich leather was overridden by the metallic coppery smell of fresh blood.

Matteo sat rigid in the leather seat, holding the young woman against his chest. Her small frame felt impossibly light almost delicate like a porcelain doll crushed beneath an iron boot. Every sudden turn of the high speed vehicle sent a tremor through her body, causing her to whimper softly in her unconscious state.Push it faster, Tomas, Matteo commanded, his voice cold, razor sharp, cutting through the low hum of the engine.I'm doing eighty on a flooded mountain road Don Matteo, the driver replied, his knuckles white on the steering wheel as he checked his rearview mirror. If we hit a patch of standing water at this speed, we won't make it to the hacienda alive. Then we die trying, Matteo snapped, his dark eyes never leaving the girl's face. Faster. In the front passenger seat, Esteban turned around, his eyes filled with disbelief and growing paranoia. He held an encrypted satellite radio in his hand, his eyes flitting between his boss and the bleeding civilian wrapped in Matteo's expensive wool coat.Boss, Dr. Vargas is setting up the trauma bay at the estate, Esteban said, his tone careful, walking a tightrope between absolute obedience and deep concern. But this is crazy. If the police find out a hit and run victim was taken to the Restrepo compound, it gives the Attorney General the exact search warrant he's been praying for. Not to mention the rival spotters in the valley. If Cali finds out 'El Hielo' has a soft spot, they'll burn the valley to get to her.Matteo didn't answer immediately. He reached down, his heavy, scarred fingers pressing gently against the side of her neck, searching for her carotid artery. Her skin was freezing, slick with rain and mud, but beneath his fingers, he felt it a weak, rapid, fluttering pulse.Thump. Thump. Thump. She stays with us, Matteo said, his baritone leaving no room for argument. No hospitals. No police. No public records.Who is she, Matteo Esteban pressed, dropping the formal title in a rare moment of desperation. You said back at the ditch that you knew her. Who is this girl.Matteo stared at her pale features. The high cheekbones, the delicate curve of her nose, the small notch in her chin. He remembered the intelligence file his men had gathered seven months ago during the violent purge of the rival Norte del Valle remnants. He remembered the photograph pinned to a wooden board in a secret war room a photo of a smiling young university student holding hands with a young man whose face had eventually been crossed out with red ink.Her name is Isabel Matteo murmured, his thumb unconsciously wiping a smudge of dark mud from her cheek. Isabel Rojas.Esteban's breath hitched. His eyes widened in absolute shock. Rojas? As in Santiago Rojas? The same Matteo said darkly.Jesus Christ, Matteo Esteban swore under his breath, turning entirely around in his seat. Santiago was the head logistics lieutenant for the Cárdenas clan. He was the one leaking our drop zones in the Caribbean The man who almost got us killed in the coastal ambush You ordered his execution yourself six months ago His sister shouldn't even be alive she was supposed to be in Bogotá at school. She wasn't supposed to be on a pitch black mountain road in the middle of a torrential storm either Matteo growled, his eyes flashing with a dangerous, lethal intensity that made Esteban instantly shut his mouth. Yet here she is. Under my wheels. Dying in my arms.It could be a trap Esteban whispered fiercely. What if Santiago's people set this up? What if she was planted on that road?Look at her Esteban Matteo barked, pointing down at her broken, shivering body. Nobody orchestrates a trap by throwing an unarmed woman into the path of a four ton armored Mercedes traveling sixty miles an hour down a mudslide It was an accident. A cruel twisted stroke of fate. The convoy suddenly veered sharply to the left, passing through heavy electronic steel gates hidden deep within the forested hills of El Poblado. The iron gates swung shut behind them with a heavy deafening clang locking out the storm and the rest of the world. The vehicles surged up the long, winding cobblestone driveway of Hacienda Las Calas, a sprawling, fortress like estate surrounded by twenty foot security walls, high definition thermal cameras, and armed guards patrolling with automatic weapons. The moment the SUV screeched to a halt beneath the covered portico of the main house, the doors were thrown open.Dr. Vargas, a tall, gray haired man who had served as the Restrepo family's private physician for two decades, was waiting with two armed tactical paramedics and a stainless steel gurney.Get her inside Now Dr. Vargas ordered shouting over the sound of the pouring rain that continued to hammer the terracotta roof.Matteo didn't wait for the paramedics. He scooped Isabel into his arms stepping out of the vehicle and striding directly into the grand foyer of the mansion. White marble floors reflected the warm light of crystal chandeliers instantly stained by the trails of mud and blood dripping from their clothes.Take her to the East Wing medical bay Matteo commanded his voice echoing through the massive hall. They rushed through the wide hallways into a fully equipped, state of the art trauma room hidden behind a concealed door in the wing. It was a room designed to treat gunshot wounds and shrapnel injuries from cartel warfare stocked with surgical tools blood bags and advanced monitoring equipment.Matteo laid Isabel down on the cold surgical table. As he pulled back his coat, Dr. Vargas moved in quickly, cutting away the torn, muddy yellow dress with surgical scissors to assess the damage.Severe blunt force trauma to the chest, Dr. Vargas said rapidly placing a stethoscope on her pale skin while a paramedic attached a heart monitor. Possible fractured ribs, deep contusions, low body temperature, and heavy blood loss from a laceration on her scalp. Her vitals are dropping. The heart monitor began to beep frantically its rhythmic tone spiking into a erratic alarming sequence.Beep Beep Beep beep beep...

She's slipping into neurogenic shock Vargas called out, pulling a syringe from a tray. I need two units of O-negative blood, now Clear the room, Matteo You're in my light.Matteo didn't move. He stood at the head of the table, his tall, imposing frame casting a long shadow over the medical team. His hands, usually so steady when holding a gun were subtly trembling. He reached out and grabbed Isabel's cold limp hand in his own.Save her Vargas Matteo said, his voice dropping into a dangerous, terrifying whisper. If she dies on this table I will personally burn your medical license and bury you in the valley.Matteo, get out Vargas yelled back, showing no fear only because he had known Matteo since he was a boy. Let me do my job.Matteo hesitated for a fraction of a second. He looked down at Isabel's face. Her lips were pale, tinted blue from the cold storm. Her dark lashes rested against her cheeks.Suddenly, her body convulsed violently on the table. Her eyes snapped open not fully, but enough for the harsh fluorescent lights of the operating room to catch the striking, emerald green color of her irises. She turned her head slightly, her gaze locking onto Matteo's dark hollow eyes. She couldn't see clearly through the haze of shock, pain, and blood, but she felt the crushing, warm grip of the hand holding hers. With a final desperate surge of consciousness Isabel's fingers tightened around Matteo's wrist her nails digging into his skin.Santiago she gasped out, a tiny trail of blood running from the corner of her mouth. where is Santiago. The sound of her brother's name hanging in the sterile air sent a violent chill down Matteo's spine. Before he could answer her grip went limp. Her eyelids fluttered shut her head fell to the side and the heart monitor flatlined into a continuous, piercing tone. She's in cardiac arrest Vargas yelled, grabbing the defibrillator paddles. Charge to two hundred Clear Esteban grabbed Matteo by the shoulder, physically pulling his boss away from the table as the medical team rushed in around her. Matteo allowed himself to be dragged back toward the heavy double doors his eyes fixed on the pale girl dying on the steel table.

His hands were covered in her blood. The blood of the sister of the man he had murdered.

As the heavy wooden doors swung shut, sealing him out in the cold silent hallway the piercing sound of the flatlining monitor continued to echo inside his head promising a war that no bullet could ever win.

Chapter 3 The sanctuary of guilt

The sharp smell of antiseptic and rubbed alcohol clawed at Isabel's senses before the light ever did.

She woke slowly emerging from a dark suffocating abyss where the sound of roaring water and metallic crunching tore through her mind in an endless rhythmic loop. Her body felt impossibly heavy anchored down by a deep throbbing ache centered in her ribs and head. Every shallow breath she drew felt like swallowing crushed glass.

Her eyelids fluttered open Above her the ceiling was not the yellowed water stained plaster of her small apartment in the city nor was it the sterile drop tile ceiling of a public hospital. It was made of massive hand carved mahogany beams running across pristine ivory plaster illuminated by the warm amber glow of recessed wall sconces.

She tried to sit up but a sharp spike of agony flared through her chest forcing a ragged gasp from her throat.

Do not move, a calm deep voice ordered from the shadows near the window. You have two cracked ribs and a severe concussion. Any sudden movement will reopen the stitches on your scalp.

Isabel turned her head slowly her vision blurring before coming into sharp focus.

Standing near a set of heavy floor to ceiling silk curtains was a man. He was tall broad shouldered and dressed in a crisp black dress shirt with the sleeves rolled up to his forearms, revealing dark ink tracing up his skin. His dark hair was meticulously combed back highlighting a hard angular jawline covered in light stubble. But it was his eyes that caught her hueless, intense, and piercingly cold staring at her with an unreadable depth.

Where where am I? Isabel managed to rasp out her throat dry as sand. Is this a hospital?

It is a private medical wing the man replied, taking slow, deliberate steps toward the foot of her bed. He walked with the fluid silent grace of a predator. You had an accident. A severe one.

An accident Memory returned to her in fragmented violent flashes the blinding storm the slick mountain road the blinding halo of white headlights charging through the dark and then the crushing terrifying impact.

She looked down at herself. She was wearing a soft silk pajama set and a thick white bandage was wrapped securely around her ribcage beneath the fabric. An IV line was taped to the back of her left hand snaking up to a bag of clear fluid suspended on a metal pole.

You hit me, Isabel said her voice trembling as the realization settled in. She looked up her emerald eyes locking onto his.It was your car.

It was, the man admitted without a trace of hesitation or defensive pride. The storm caused my vehicle to lose traction on the descent You stepped out from the foliage directly into my path. My driver could not stop in time.

Isabel closed her eyes trying to piece together the shattered timeline of that night. I was running I was trying to find She stopped abruptly her hand flying to her throat as a wave of cold panic washed over her. My brother. Where is my brother?

The man's expression did not alter but a subtle almost invisible tension entered his shoulders. He pulled a heavy leather armchair up to the side of the bed and sat down crossing one leg over the other.

You were alone on that road Isabel he said softly.

The sound of her name on his lips sent a shiver down her spine. How do you know my name? I didn't give you my identification.

Your wallet was in your handbag, he said smoothly telling a half truth with practiced ease. Isabel Rojas. Twenty two years old. A student at the University of Antioquia. It was not difficult to identify you.

Did you call the police? Did you contact my family? She pushed herself up slightly despite the pain her breath coming in fast panicked shallow bursts. My brother, Santiago he hasn't answered his phone in days. I was looking for him I need to call him.

Matteo sat in silence for a fraction of a second watching her chest heave against the tight bindings. The mention of Santiago's name cut through him like a blade but his face remained a mask of stone.

Your phone was destroyed in the crash Matteo stated quietly. And as for the authorities no. I did not involve them.

Isabel frowned confusion overriding her pain. Why? Why wouldn't you take me to a public clinic? Why bring me here?

Because a public clinic in Medellín is a death trap for someone in your condition Matteo said leaning forward slightly resting his forearms on his knees. They lack basic surgical supplies the staff is overworked and you would have waited four hours on a gurney in a hallway while your chest cavity filled with fluid. I have the resources to provide you with top tier twenty four hour care. You are alive right now because my doctor spent three hours stabilizing you in this room.

Isabel stared at him trying to reconcile the terrifying cold authority he radiated with the fact that he had saved her life. Everything about him screamed danger the tailored luxury of his clothing the silent military like discipline of the room and the lingering sense that this house was far more than just a home.

Who are you? she asked, her voice barely above a whisper.

My name is Matteo, he said simply. Matteo Restrepo.

The name meant nothing to her. She was a quiet university student who spent her days buried in books and her nights worrying about her brother's mysterious absences. She didn't know the dark political hierarchy of the city's underbelly nor did she know that the man sitting beside her was the most feared cartel lord in the department.

Thank you for saving me Don Matteo Isabel said quietly the heavy strain of exhaustion creeping back into her limbs. But I need to leave I have to find Santiago. He's in danger, I know he is.

You are in no condition to walk let alone search the city for a missing person Matteo said, his voice dropping into an authoritative tone that demanded compliance. You will stay here until Dr. Vargas clears you You will be provided with everything you need food, medicine, rest.

I can't just stay in a stranger's house, Isabel protested trying to swing her legs over the side of the mattress.

The moment her feet hovered near the floor a dizzying wave of vertigo struck her. The room spun wildly and her strength vanished. She swayed dangerously falling forward.

Before she could hit the floor Matteo was there.

In a single fluid movement his arms wrapped securely around her waist and shoulders catching her effortlessly. He pulled her flush against his chest lifting her back onto the bed. For a brief suspended second Isabel was pressed close to him She smelled the scent of dark cedar expensive leather and a hint of faint lingering rain on his skin.

His eyes locked onto hers inches away. Up close she could see the subtle gold flecks surrounding his dark pupils and the intense possessive grip of his hand resting against her lower back.

You do not listen very well Isabel he murmured his voice low and vibrating against her skin.

I don't belong here she whispered, her heart hammering wildly against her ribs not just from pain but from the overwhelming, terrifying proximity of the man holding her.

You belong wherever I can keep you alive Matteo replied, his thumb brushing over her cheek in a gesture that was shockingly tender almost desperate before he pulled back and straightened his coat.

He stepped away from the bed walking toward the grand double doors of the bedroom. He paused with his hand on the heavy brass handle looking back over his shoulder at her.

Rest Isabel. The guards outside will bring you food and your medication. Do not attempt to leave the wing.

Guards? Isabel called out her voice rising in alarm. Why do you need guards?

Matteo didn't answer. He slipped out of the room and pulled the heavy mahogany doors shut behind him.

A second later Isabel heard a distinct, chilling sound echo from the other side of the wood.

Click.

The door was locked from the outside.

Isabel stared at the locked brass handle the warm light of the luxurious bedroom suddenly feeling less like a sanctuary and far more like a gilded cage.
